About this course
This module will provide nurses with the skills to provide case management to COVID-19 positive patients from within a vulnerable population or marginalized group using telehealth options.
Objectives
- Define case management and care coordination
- Define the scope of practice: roles and responsibilities of the Nurse Case Manager
- Describe the importance of communication between Nurse Case Manager and other providers
- Identify groups that have been marginalized, and how telehealth can benefit them
